About this protocol
ViP MyoCardio is the cardiovascular companion to the ViP Protocol, addressing the aerobic deconditioning and elevated cardiometabolic risk that the core programme doesn't cover. It pairs a graduated daily step program (2,000 → 10,000 steps over 24 weeks) with walking-based HIIT using heart-rate monitoring, across three phases: Aerobic Foundation, HIIT Introduction, and Optimization. Adapted from the Bartlett (2018) walking-HIIT model in inflammatory arthritis and the Andreasson (2025) HIIT-in-IIM randomized controlled trial.

Evidence base
ViP MyoCardio is derived from systematic review and randomized controlled trial evidence in IIM with cardiovascular deconditioning. Protocol parameters — phase structure, load progression, and outcome measures — align with EULAR, ACR, and ASAS exercise-therapy recommendations. Outcome tracking favours open-access PROMIS instruments where validated equivalents exist.
