How it works

The complete virtual care pathway — from first contact to programme completion and PSP reporting.

Phase 1 — Intake

Referral or PSP enrolment

Patients enter FlareForward through their rheumatologist's referral or directly via a pharma Patient Support Programme enrolment. The care coordinator is notified within 24 hours.

Welcome call & eligibility screen

A FlareForward care coordinator contacts the patient within 48 hours. A brief eligibility screen confirms the patient has no absolute contraindications to supervised exercise therapy.

Phase 2 — Assessment

Baseline ePRO completion

The patient completes a baseline set of validated ePRO instruments relevant to their condition: PROMIS Physical Function, PHQ-9, Pain VAS, and condition-specific instruments (e.g., RAPID-3 for RA). These establish the functional baseline that all outcome reporting is anchored to.

Virtual functional assessment

A certified exercise physiologist conducts a 45-minute virtual functional assessment via video call. Movement quality, strength, endurance, and self-reported goals are evaluated. The assessor selects the most appropriate FlareForward protocol and individual load targets.

Phase 3 — Programme delivery

Protocol assignment

The patient is enrolled in their assigned FlareForward protocol (GRIP, VALOR, FORGE, ViP, GROUND, or BEDROCK). The 20–26 week programme plan is shared with the patient through the FlareForward app, including session schedule and home programme.

Live-virtual & asynchronous sessions

Most protocols include 3 live-virtual group or individual sessions per week, supplemented by asynchronous guided home exercises. Certified exercise therapists lead all live sessions. Sessions are recorded for patients who miss a scheduled slot.

Monthly ePRO check-ins

At weeks 4, 8, 12, 16, and programme end, patients complete abbreviated ePRO check-ins. Scores are compared against their individual baseline. If scores deteriorate or plateau unexpectedly, the care coordinator is alerted and a clinical review is triggered.

Phase 4 — Outcomes

Programme completion & final ePRO

At week 20–26, the patient completes the full baseline ePRO set again. The coordinator compiles a programme completion report: percent improvement in each instrument, adherence rate, and any clinical events during the programme.

Reporting to PSP partner

For patients enrolled through pharma PSPs, de-identified aggregate outcome data is compiled into a quarterly report aligned to the partner's medical affairs requirements. Individual patient data is only shared per the patient's consent.

Maintenance pathway

Patients who complete a FlareForward protocol receive an evidence-based maintenance home programme. Re-enrolment in a subsequent protocol is available based on clinical judgement and partner programme terms.
